How do I get my Survey comments on my report?

I have used this link to guide me through building a report to view survey results.

The part I can't figure out is how to pull the info from the text box where the user is making a text comment.

That example has it grouped by category. If you use List type reporting, just add String value to your report. That will show you any comments they leave:
