How do I remove the Request Approved from start of workflow

I have a workflow that has a “Waiting for an approval” stage as the first item after Begin in the workflow then it moves on. However when you submit the catalog item associated with this workflow and check the stages it always says Approved as the first stage. I don’t have any stage that says Approved in this part of the workflow.
I tried changing the order in the Workflow stages on the Workflow properties but it didn’t make any difference.
How do I remove the Approved stage from the start of the workflow?

Basically, it seems a lot of people are having trouble with this and there's no really good way to deal with it.
