Can someone please help me on the below requirement.

How to add the Note message under the Add Attachment.

I want to add this message " 

Request will be rejected if the documents are not uploaded" under the add attachment.

I have tried the solution but it showing as a variable attachment i don't want the variable i need only the message need to visible under the Add Attachment.

As highlight in the below image i need only the message to be populate.
