How to add Resolution Notes on the Incident Form

In my personal development instance there is a tab for Resolution Information.  Using the Form Design tool, I have been able to add this tab back and all the fields but the Resolution Notes field.  It shows it is the "close_notes" field from the task table.  Where do I find this field in the Form Design tool?

Figured out why the close_notes was not showing in the Form Design tool, the field was marked as inactive in the Dictionary.  Activated it and it showed up in the From Design tool.

I do not see a Close Notes option either.

As you mentioned below, you deactivated the field, thus causing the issue where you couldn't see it for selection in the form designer.

The 'close_notes' field from task table is the correct field to use in Form Design tool, here is a screenshot of the OOTB configuration for that field:
