where should the user be taken when they click on the Survey?

I agree with @Community Alums.

What's the business case to show that as link?
it makes sense showing the INC as link so that user can click and navigate to that record

If you still want to do then you will have to update the OOB widget

This can't be done and I agree with @Community Alums that is more on education not on configuration. Also what value it drive by adding underline?
