How to align check box and single line text in service catalog

Is there a way to align the checkbox and a single line text next to each other as per the below screenshot in Service Catalog form.

Not that I know of. The best you can do is to have a check box and a text box on a single row. On the other hand what is the use of the check-boxes? Isn't the filled state of the text box the same as checking or unchecking? I mean if someone needs to add - e.g. - Change group info, that someone should just fill in the field. Or if that someone does not need to add a Change group, that someone should just leave it blank. What would be the purpose/usefulness of having the check-boxes too?

The requirement is to update the table entries on submission of a catalog.

We have a textbox to enter the server name and we check the checkbox of the field and enter the new value on the single line textbox associated with that checkbox and submit the catalog.

So basically we are looking for updating the table entry of a field value on submission of a catalog.

If someone leaves the checkbox as blank then that field value will not be updated.

So if someone enters a Server name, but does not check the box, you will discard the value?

I mean what is the difference between someone filling in Server name and ticking the box and someone filling in Server name and not ticking the box?
