How to create a change ticket from a service catalog item

Hello,
I'm trying to create an item in Service Catalog. This item will be used to ask support team to configure specific access point.
The idea is that operationnal will request the item and fullfill all information (for example name, SSID, site, ...) and all this information must be used to automatically create and complete field of a standart change ticket.
I have already test this solution https://www.servicenow.com/community/itsm-forum/standard-change-templates-used-in-flow-designer/m-p/... but it doesn't work (I think i've missed something).
Is there somewhere a tutorial to help me for creating this type of item ? In serviceNow learning I don't found any learning explaining how to do this. I'm not sur to understand if there is a difference between flow for service catalog and flow for app ?
