Hello @Sathwik1
This was the most awaited question I was looking for, I too came across this scenario where I spent plenty of time to under why it was happening, finally it's a simple thing.. just inactivate the OOB Risks and set the thresold to "1". It will work perfectly.
Please mark my answer as both HELPFUL and CORRECT, if it helps.
Regards,
Raghu.

Hi @Sathwik1 ,
If you talking about the Change record Risk conditions, then it will be applicable for risk assessments as well.

Hi @Sathwik1 ,
There are two methods to calculate the risk of a change. The Best Practice - Change Risk Calculator is activated in the base system by default. Change Management - Risk Assessment is optional.
- Change Risk Calculator uses predefined properties and conditions to calculate a risk value.
- Change Management - Risk Assessment uses information provided by the end user to assess a risk value.
Now, If both Risk Assessment and Risk Calculator are active but you want to use only one method, remove conditions for the method you do not want to use.
- Navigate to All > Change > Administration > Risk Assessments.
- Open your Risk Assessment Record
- Remove the Condition
- Save

Tried already, I am don't think directly removing the conditions on Risk Conditions will not help, already tried it..
There are 4 Risk Conditions available OOB
3 are with some conditions and 1 is default..when all the three conditions didn't satisfy then it will proceed with fourth one i.e.,, Default one.
So if I remove conditions for all 3, then Default will work..
