What is best practice to ensure a notification is sent to the requestor when an approval is rejected, and to include the rejected comments? OOTB is set to approver only, which is strange as you would expect the requestor to get notified of this with the comments.

There is a 'Change Rejected' notification on the Change Request table OOB, you can modify that to include the requestor.
If you're only concerned with the Approval being rejected and not the change as a whole, you can expand the 'Approval For' field and select the 'opened by' field, in most implementations I've seen this tends to be the same as the requestor.

Hello Simon,
Have you used approval activities in the workflow?
If the approval rejected then you can configure to the required users
