How to trigger each surveys notification to multiple groups

Hi,
I have two surveys (Demo1 and Demo2) configured to trigger based on different assignment groups (abc and xyz). While the survey notifications are sent correctly, the survey link in the email preview is referencing the same record (HRC00...) even for different cases, and the user sees incorrect names when accessing the survey.
Issue 1:
Survey name: Demo1
Trigger conditions: State is Closed Complete, Assignment Group is abc
Problem: When the survey is triggered, the email contains a correct survey link, but clicking the link shows a static record (HRC00...) instead of the actual case it was triggered from.
Issue 2:
Survey name: Demo2
Trigger: State is Closed Complete, Assignment Group is xyz
Problem: Survey is triggered, but when previewing or opening it, it shows the same user name or previous survey name every time.
Can anyone please help on this, It will be gratfull.

Unable to reproduce this issue in my PDI, so requesting you to share the screenshots of your survey defintion (Table: asmt_metric_type) or Navigate to All > Survey > View Surveys
Also, try to create one dummy survey as shown below to see if it is working or not:

I created the trigger condition like below:
One survey is triggering twice.

In the trigger condition don't use IS operator, Please use Changes
Multiple Survey Instances Generated
Something like this
State Changes To [Close Complete]
AND
Assignment Group IS XYZ
