Image Scale in Surveys

I have tried Outlook Actionable Message (OAM) to implement the ability for users to answer surveys within their emails but can't seem to get this working.

How do I add the ability to click 1 image in the email and automatically send the image scale response to SNOW?

@Jay Freise I didn't find any docs stating the image scale support in San Diego. I also tried this in an upgraded instance but still do not allow me to save . The same erorr is returned 'Image Scale type question is only compatible with one question survey. To support Outlook Actionable Message, please either make sure current survey has only one question or remove image scale question from your survey'.
