Hello Team,
Can anyone please let me know whether it is possible to include the wordings instead of numbers in Radio Button (Variable Type) in Surveys?

So one way of doing this would be to use the multiple choice variable type instead of numeric scale.
You will need to add your values and labels individually in the Question Choice related list ...
Once you have your choices defined you can try the variable ...
If you want the layout to look horizontal you'll need to (1) add Choice direction to the variable form by personalizing the form layout
... and (2) add Multiple Choice to the "Hide Choice Direction" UI Policy by personalizing UI Policies from the Variable form ...
Test the variable ...
