I found that allowing "Enable email sending and receiving" in the "Manage Email Properties" dialog of your PDI managenent panel is necessary before the MFA via email is sent.
Some other action on the PDI might be necessary before anything is sent at all.
Suggestion to ServiceNow: Would it be possible to reset the admin's entry in table
user_multifactor_auth if we select "reset instance password" in the management panel
to generate new instance login credentials?
Then we could recover more easily from MFA issues.

i tried to approve app and google authenticator to login to my instance : dev181160 and kept failing and was not able to receive the code at my email . @Randheer Singh
