Transform map record not provided-error while importing data

Hello,
I get this error when I do data import from CSV files:
"Transform map record not provided, transform stopped: no thrown error"
Any idea about why it happens?
I have some records in ServiceNow. Users attach files to that records and then I trigger imports with a UI action. Transform works for some records and import data from attached file but for some records, I get this error.
Anybody has seen this error before ever?

We had the same issue, but for us the Transform map associated to the data source was marked in active probably had done it by mistake. Once re-activating the transform map, it ran successfully
