Trigger Survey : Flow Designer or Best Practices Use Case

Hello Experts,

I want to trigger a survey for a user based on the below condition.

  1. User must have used servicenow in the last six months.
  2.  User have created incident or request with in that 6 months time frame if not skip the user.

Survey should be triggered only once in six months for user. Please help.

Is it easy to use servicenow for reporting the IT related issues(rate scale 1-10)?

This is a pretty standard use case for Assessments, but it's a complicated module.
