Unable to Attach Files to Catalog Item

We have a new hire form that sometimes requires a file to be attached and have been using the form for years.
Recently we have had a couple of users that are no longer able to attach files but were able to attach in the past.
Any suggestions would be greatly appreciated,

Tough to find out the exact cause in this case. It can user permission issue look. Try to compare that user with you and see any changes?

After additional testing we found that if you click on "Choose File" the file does not attach, but if you use "Drag and Drop" the file does attach.

I am exploring the same issue. Random event for the users; as in sometimes attachment works and sometimes not. After Requested Item is created, user can attach file that failed to attach through the catalog. Also, the attachment is mandatory on the catalog item.
