what is best way to create report to get survey response in report along with all type response

Dear All,

I am looking for to create a report to get all survey response in once report like we have one metric called - Incident surveys where we have 2 question - 

1- one question have  some emoji  where value are - happy, satisfied, very satisfied, dissatisfied

2-  we have given string field where user can write some feedback,

I have tried to create report in instance table but not getting expected results,

we want incident number, short description, survey taken on, assignment group,  both question of survey and  their answers submitted by users,

Please suggest best way to create and also share which table is best because we used many table, like metric definition, questions, instance and etc..

Hi @nitin gupta1 ,

Besides what is shared by @Tanushree Maiti , you also have the possibility to create a database view to have both score and string field together with task fields in a single row for reporting purpose.
