Opened By - This field captures the User who opened/ submitted the change request form.
Requested By - This will be defaulted to User who opened Change Request form, however this can be Modified to any other User, you can consider this as "On Behalf Of" other User functionality.

The way I always understood it is that opened by is captured in the background and is the logged in person who opens the ticket. This is not a field on the form. Requested for is who is it for. In some cases you could request for someone else. This field shows on the change form usually.
