You can create notification on Assessment Instance with following conditions

Trigger condition is definitely required if you want to trigger the Survey.

Whenever the Survey will trigger, it will also trigger the custom notification.

Just revert back BR to OOB and add "incident is resolved" to survey trigger condition, also update your survey metrics in notification's condition builder

I think you should add all the condition which i shared in screenshot.

Later you can remove according to your use cases.

@Pranesh just a small thing, I did as you mentioned now in who will receive I need to send service offering manager of that incident...it is present in incident table but actually in notification I am using assessment table how can I acheive it?

You can use "assigned to" field and dot walked to manager
