You can create notification on Assessment Instance with following conditions
- Mark as New
- Bookmark
- Subscribe
- Mute
- Subscribe to RSS Feed
- Permalink
- Report Inappropriate Content

Hi,
then you can do this
1) update OOB notification so that it doesn't trigger for your survey
2) The newly created notification should trigger only for your custom survey

I could see you have marked other response as correct.
The same approach was mentioned in my previous comment to update both the notification conditions so that they are mutually exclusive.
i.e. OOB notification should not trigger for your Survey when you update OOB notification condition
Your new notification should only trigger for your survey and not other survey when you update this notification condition.
You can only mark 1 response as correct.
So please mark appropriate response correct and helpful.

Just a small query, is there any cool down period for creating of suvery instances? because last night I tested with 7 incidents for 1st incident survey instance was created and for all other remaining survey instances was not created..
now I have tested survey instances was created and tested again now survey instance was not created.. can you please confirm what is the issue?
