Why work notes is coming twice in the activities?

we have this code in the server script of a widget.

This will do  - when a approver rejects the request he will type some message and this is assigned to work notes and comments.

But the issue is sometimes(may be like once in 10), work notes and comments coming twice. Even if we follow the same steps to reproduce the issue, it will not come , only sometimes it is happening like that.

Hi Bharath,

Is the duplication happening for both approvals and rejections?  If it's only for rejections, you are probably seeing the OOTB behavior that requires a comment when rejecting.

this duplication is happening for rejection and not every time.

As I said before its happening once in 10 times.

That's the issue
