What's your question?, if this is your story, pls go to the assessment workspace and make sure to update the template accordingly for the control attestation (Note: this uses smart assessments).

I want to add a due date and a control description in the header of grc classic attesatation temlate when user fill the attestation form.

Make sure all the fields are supported here and it also works correctly on the Service Portal.
