Creating a New Skill no "Default Provider" is selectable

I create opened Now assist skills kit and clicked "Create Skill". 

The "Default Provider" and "Provider API" fields both have no drop down options. 

Any suggestions for where to investigate this issue?

You should see a green 'Buy' button when viewing the app in the Store.  That should start the process that leads to accepting the terms.
