How can I create a New Data Visualization using an indicator source, that trends score by month

Hoping someone can help.  I've recently switched from Core UI reporting to Platform Analytics, but need some assistance.
I need to create a visualisation that shows me the % of Successful Change per month that I can filter either by Assignment Group, or Service.   
Ideally I want the visualisation to show me a single score for that month.  The indicator source that I'm using is "% of Successful Changes", which is collected daily.  
If I have 2 Changes closed in July, 1 Successful, 1 Not Successful, I'd like that to show me 50% for the month of July.  
I'd also like to show the difference from June.  For example if June Success was 100%, I'd like the visualisation to show me 50% for July -50% from June.  

Can someone please let me know how I can roll up the daily scores to a monthly score for the month that I'm reporting for? 

I'd also like to create another visualisation that shows me the monthly scores over a 6 month period, split by month.  Y Axis to show the % achieved for the month, and the X Axis to show me the months that the changes were closed. 

My question here is specific to Change Mgmt, but I also need to consider the same approach for other reports that I'm creating. 

Any help would be most welcomed.

Refer out of box indicator % high priority incidents to design your formula indicator % Change Successful

You can use time aggregation to show the AVG score value for the indicator over last month,

Similarly you can use time aggregation to provide monthly data for last 6 months
