I have the indicators marked as show real time score.

I'm then trying to create a formula indicator (to show open/close rate of incidents for the prior month) - but I still get nothing on the scorecard. I'm just trying to get a single score (percentage)

I don't believe you'll be able to get a real time score as a percentage because it will require a formula.   Real time data
