"Work notes" and "Additional comments" on report

Dear all,
I wanted to extract report where I can add "Worknotes" and "Additional Comments" along with other fields in "incident" or "Requested Item"
Can someone please let me know which table I have to choose or join which all tables etc.

I'm trying to pull report on incident and requested_item table.
I want along with other fields of incident /requested item worknotes and additional comments also.

I have the same question, hoping someone can help with this.
I also want to create a report of the number of comments and worknotes a user has created.
