is it possible to report (Audit) when a Tag was removed and by whom? and also when it may have been added and by whom?
is there a way to achieve this?

For adding, you can build reports on Label Entry table(label_entry). Not sure about deletions.

I was trying this out and got some issues and still finding it can you give some more suggestions if any thanks in advance.

Removed tags you can't really manage, either you can before delete business rule on this table, which captures it somewhere, what and who has deleted it.
Beyond that I don't think there is any other way
