Survey Trigger Condition Help: Create Survey on Ticket Creation

Hello,
I am working on creating a survey asking for feedback on our End users' experience with our Service Portal. To get the feedback while the info is still fresh in their mind, the requirement is to trigger a survey upon Requested Item Creation for the requested by (We are using RITM so we can use specific Requested Items as one of the triggers). One survey needs to be offered for every RITM creation. Knowing I cannot use the Repeat Interval and setting up a condition by status could trigger multiple surveys for a single ticket, How would I go about setting up this trigger condition?
**Bonus points if I can do this without scripting it
