I'm having one form and on that having request approval UI action once assigned to click on it approval will goto the another user once that user approved from self approval then on activity tab of form its showing approved by system instead of user name

It is difficult to say what is causing this without looking at the code.
Will you be able to share the screenshot of the activity where it is reflecting the incorrect username?
