Import Options Missing in Create Entitlements

I recently requested a new Washington DC PDI and installed SAM Pro plugins from ServiceNow including Software Asset workspace. Using the workspace when I go "Create Entitlements", the options to select a method are missing:

I tried this on a different browser as well. No difference.

 We have the same problem in our demo instance for customers and in a customer's instance. Only way to add entitlements is through the classic view under Software-Entitlement Imports.
