How to configure the Request Exception form in popup to add customized fields on it?

Hello All,

I want to configure and add the customized fields on the 'Request Exception' form which opens as a popup when Request Exception button is clicked on a Remediation Task. Can anyone please guide me for the same?

You will need to customize the UI page "review_request" by adding your required fields to the HTML and add processing for those fields on the Client Script.
