Can someone share how you use post implementation reviews for failed changes? Please share screenshots and instructions on setting it up. Thanks

OOTB, PIR task generated only for Emergency changes in SN. You can create/ modify the flow to generated it for all changes (but not recommended)
The usage of PIR is mainly depend on your customer and business and IT processes.

5Why's
Put quuetsions like
Conducting a PIR soon after implementing a change enables you to answer insightful questions such as:
- Did the change fully solve the problem that it was designed to address?
- In the case of failure, did the back-out plan work correctly?
- What impact did the change have on customers?
- Did we allocate resources effectively throughout the process?
- Was the change implemented on budget, and in a timely fashion?
and much more. YOu can get corrective and preventive action from PIR.
