default ux form header

how to prevent default ux form header of itsm application getting applied to the form record in the another workspace in servicenow.can anyone help on this...

if you see in image below sc_req_item form header is for 3 different workspace

there is an active field, you can mark that as inactive and create your own in the workspace you want

i already did still ITSM default one is getting reflected not the new one which is created for my workspace in custom application

did you deactivate the OOTB one?
