I want to display the choice of 'apply_address_old' column at catalog item. Just like this example. How can I do it?

Just confirming, Do you want to add "lookup multiple choice" type variable over catalog form?
If yes then what table you want to add there ? if you select that table then its not showing it?

Correct me if i am wrong, Seems like you are expecting the layout should have gap between choices?
if yes then it does not work like that way, you can either move Across or Down the choices, if you are expecting to have some gap between choices then it wont work.
