Add a radio button to a form (not a catalog page)

I can't believe this is as hard as I seem to be making it, but all I want to do is add a radio button with three choices to my form. I notice that there is no such control out of the box even if radio buttons are HTML 101. I have read about turning on the hidden radio button in sys_glide_object and I've used it in my PDI, but it is ugly (see image). I'd like to be able control the number of rows & columns (I have other radio buttons with more choices).

I see that there is a UI Macro called ui_radio, but I don't see any documentation on how to configure it and get it onto my form. I know I can get the same effect using a dropdown, but I think that radio buttons make more sense in this case and they are a basic HTML element. And, as should be obvious by now, I am a relative ServiceNow newbie (but an old hand at building web-based systems).

 

Any help would be greatly appreciated.
