Cancelled : error="Failed to send process plan with ID: to instance

Lately after we upgraded our instance to Tokyo, we get the following error on executing the Flow.
Cancelled : error="Failed to send process plan with ID: xxxxx to instance. com.snc.process_flow.exception.ProcessAutomationException: Unable to serialize process plan to JSON"
The action within the flow gets completed fine in the backend but fails as above. This action within the flow actually worked fine before the upgrade. Has anyone experienced the similar issue and has any resolution for this? Any help is highly appreciated.
