Do most companies send request surveys on the requested item table or sc task table?

We currently send the survey out when the Requested Item is complete but of course we don't have the Assignment Group/Assigned To information since that's on the sc_task table and could be assigned to different teams.  We just wanted to see what other companies were doing as far as what table are the request survey's being sent from.

Hello, It will be on Requested item table only, as survey is sent for requested for hence mostly requested item tables will be used. I suggest you populate assignment group and assigned to information accordingly so that it will be helpful in reporting purpose. Mark my answer as correct if that helps. Regards

Agreed. Survey should be done on Requested item.

OOB a user without roles (end user) does not have access to the sc_task table. The tasks created there are meant to provide tasks for a user with fulfiller role (itil) to complete.

You can compare it to the Incident table, which could have incident tasks connected. The end user will see and react to the incident record, not to any incident tasks that are connected to it. So when the incident is completed, any survey that is sent is in regards to the incident, not to any incident tasks.
