How to add help text to the survey question?

Hi All,

How to add help text to the survey question?

For example i have survey questions  with number scale type.

For each questions  need show help text like  5 for good,4 for best,3 for average.

Give a below try and mark it as helpful if it works for you.

1. Go to the Design Survey section of your survey.
2. Click a question or additional text field.
3. Highlight the text you want to add a tooltip to.
4. Click the Tooltip icon in the formatting toolbar.
5. Enter your tooltip text.
6. Click OK.
