How to create an icon next to a caller field which displays caller information in incident form

Need to create an icon next to a caller field in the incident form and it should display the details of a caller

please correct me if i am wrong,

the "i" icon you are showing on the screenshot you want to show on which field ?  that icon will automatically appear on reference field.

eg: if you see the caller field that icon came because you have selected the value there.

you have to create here UI Macro and then call that ui macro inside the field attribute .

right click on caller id field and click on dictionary 

then check attribute field and send me the screeshot
