please correct me if i am wrong,
the "i" icon you are showing on the screenshot you want to show on which field ? that icon will automatically appear on reference field.
eg: if you see the caller field that icon came because you have selected the value there.

you have to create here UI Macro and then call that ui macro inside the field attribute .

will you send me the screen shot of ur caller_id attributes
right click on caller id field and click on dictionary
then check attribute field and send me the screeshot
let me have a look once what is missing
