HRI Case User Acceptance workflow not triggering for some HR Cases

In an issue we're struggling to reproduce, we have seen several HR Cases recently which get set to Awaiting Acceptance but do not auto close in two business days, as expected once the HRI Case User Acceptance workflow timer has completed.
On further investigation, this workflow is not being triggered on impacted Cases when they were set to Awaiting Acceptance. We haven't modified this workflow and 99% of Cases are functioning as expected.
Every attempt to replicate results in the HRI Case User Acceptance workflow triggering once the state hits Awaiting Acceptance, which can occur via manual selection, list editing, or via after final task completion.
