PowerShell Orchestration Activity running twice on two different MID Servers at same time.

Hello everyone, 

We have various PowerShell activities automated through Orchestration and recently (not sure when) I started to notice that few of them are running twice, so the instance is sending the PowerShell activity to two different MID Servers at the same time. 

Some of the logs were already cleared because more that two weeks already passed, so I'm a little lost where to look.

Any clue will be helpful.

EDIT:

I could narrow down to the approval activity, somehow, is right after the approval, all subsequent activities get duplicated!

We can close this topic.

I've isolated the issue to some corrupted update sets back in January this year.

We will have to re-work the approval activity.
