Sequential Flow Designer Approvals

I am trying to create a flow which will perform sequential approvals for a record, however after the 1st approval approved it changed back to "Requested" together with the 2nd approval.

1st approver is Manager

2nd approver (3 groups) - approval group will trigger based on legal entity

Can someone advise what is the problem?

so are you saying 1st approval is generated and was approved and when you generated 2nd approval the same approval record moved from approved to requested?
