Service Catalog requests missing mandatory fields

We are facing multiple scenarios where users are able to submit request via the service portal without populating mandatory data.
When we tried to replicate the issue with the same user the request could not be submitted without entering the same mandatory fields.
Has anyone faced this issue in your SNOW instances, and can share me if there are any root causes identified for this issue?

Similar to what @Luiz Lucena mentioned - check your UI Policies on the Catalog Item(s).
Another idea is if you have the 'Bundles' functionality active for your portal. Bundles basically takes a snapshot of the Catalog Item at the time when it was accessed and saves a copy of it for an end-user and doesn't receive any modifications/enhancements that you might make to the Catalog Item / Flow after the point when the end-user adds it to a Bundle.
It's a frustrating functionality that we've been discussing turning off.
