Installed Solarwinds Alert Integration in SNOW.

In the above url, #5 looks different for us. I don't see any incident fields under this section.
Does anyone know how to bring the Incident fields here?

Sure. But we are using the trial one, so Solarwinds not able to open a case with us. Anyways will speak to them.
