What steps should be taken when decommissioning a server?

Obviously change it to Non-Operational and Retired but what other information should be cleared out, if any? Should Decommission be added to the name of the device?
I would like to know what others are doing for their process.

What we are doing is retiring the server, and then excluding the IP from the discovery schedule. It's kind of a maintenance nightmare , but at the same time, it makes sure that we are not getting dinged for this asset in our discovery licensing. As I understand it, ServiceNow says if the device is in a discovery schedule, and is in the cmdb, it counts as a license. I didn't see anywhere in there where it indicated that a "retired" server didn't count as "in the cmdb", but better safe than sorry I guess. Probably overkill, but until I can get a 100% straight answer on the licensing, we will continue to do that.
