Yellow work notes field

What would cause a user's work note field to turn yellow and lose the post button?  A note can be saved if the user enters a work note in the yellow field and saves.  But the user cannot see the worknote.

I had this issue with a user before, I had to remove all their records in "sys_user_preference" (User preferences), have them log out and back in and then everything was back to normal.
