Hi @EdmarM
Ensure You have added choices for your custom field request_type in problem form. and the Choices are ACtive.
- Right click on Request Type field in Problem Record form ->Click Show choice List
- In the Choice table field, select the table as problem
- In the Choice field, select the request_type field
- Check if Active choices are there
