Create a button if tested is equal to Dead

Hi everyone,

I want to create a button using ui action on the custom table form, the condition is , if I chose Tested is equal to Dead, a button should be created else no button should create, After clicking on the button record should be save.

i wrote ui action script and onChange sciprt , I am getting some errors and code is not running.
