SCTASK not getting in a RITM.

Hello,

 

I received an incident stating that for a particular RITM raised under the catalog item-Leaver/Offboarder, RITM cannot be  closed due to missing SCTASK. I created the request for same catalog item in UAT and it is creating SCTASK there. I checked the flow used here. Seems to be same in both UAT and prod. I also impersonated the user and created the task in uat then also the SCTASK is created. I checked for few roles as well. seems to be fine. I do not know where do i check and troubleshoot this to investigate further. when i checked the flow context under related list of the RITM  in the PROD it is showing 'Artifact not found' but whereas in UAT it showed the execution details.
