How do I add the trigger id's assignment group to this assessment instance form

Good morning or afternoon everyone!!!

I was trying to figure how to add this field to the assessment instance for surveys so I can run a report to show the assignment group. How do I add the trigger id's assignment group to this assessment instance so can add it for my report? Also, make is visible only for people who get the ticket in the back end
